Supplemental Conflict Reporting Form

After filing a financial disclosure in the Business Relationship Reporting system ("BRR"), employees may submit this Supplemental Conflict Reporting Form to request a conflict of interest review by the COI Committee to determine what steps are needed to manage known or potential conflict of interests in the following University activities:

  1. Unsponsored (or internally funded) non-human subjects Research (i.e. research which is not administered by the Office of Sponsored Projects or the Institutional Review Board)
  2. Scholarly or Educational Activity
  3. Business Transactions including, but not limited to, purchases, contracts, subawards, license negotiations, lease agreements, etc.
